Bridge House Care Centre

Bridge House Care Centre provides personal and nursing care for up to 35 older people, including those living with dementia or other physical and mental health conditions. Staff support people to have maximum choice and control over their lives while maintaining their safety and wellbeing. Care plans are regularly assessed and monitored, incorporating professional advice with clear guidance for staff to follow. The home ensures people's health care needs are met, with support for attending appointments and access to a choice of food and drink to reduce risks of malnutrition and dehydration.

The premises are adapted for people with dementia to help with orientation around the home. The newly appointed registered manager has implemented improvements including enhanced quality checks and audits covering staff support, medicines management, and health and safety. The manager communicates regularly with people, relatives and staff, actively gathering feedback to monitor and improve the service. The home works closely with other health and social care professionals to support residents.
