Brackley Fields Country House Retirement Home

Brackley Fields Country House provides accommodation and personal care for up to 32 people, including younger and older adults and those with dementia. At inspection, 24 people were using the service. Inspectors found significant concerns with record-keeping, noting gaps in documentation of personal care tasks and risk strategies. People's records did not consistently evidence that person-centred care was being delivered, and not all risks were clearly identified and properly mitigated. Injuries had not always been recorded with limited follow-up information provided.

Medicine records also required improvement. While people received their prescribed medicines, documentation did not always explain why 'as required' medicines had been given, and staff lacked the information needed to understand when such medicines should be administered. The home's audit systems were not effective at identifying missing information, and overall systems and processes for oversight and continuous improvement were inconsistent.
