York House accommodates up to 13 people requiring personal care, with a particular focus on supporting those with mental health needs. At inspection, all 13 places were occupied, with three residents requiring personal care support.

The inspection identified significant concerns regarding safety and care quality. Risks to people had not always been properly assessed, and staff failed to consistently report incidents. The registered manager had not ensured adequate staff training in medicines administration, and competency checks were not completed. Additionally, people were not consistently supported to have maximum choice and control over their lives, with staff not always supporting them in the least restrictive way possible or in their best interests.
